At the heart of Vivabrook’s services is our Concierge Care Team

Our cohort of trained and licensed nurses including caregivers are carefully vetted and recruited for their chosen vocation in providing compassionate care for seniors and tending to their individual needs and preferences. Our team members are employees  as well as partners in the cause.

Vivabrook’s Smart Hub

Our care professionals  are responsible for enabling seniors to age optimally at home.

Vivabrook’s Smart Hub is our nursing station, operated 24/7 by our Concierge Care Team. The activities of our seniors are remotely monitored through our Smart Hub. We reach out regularly to seniors; remind them to take medication when necessary; schedule their activities including medical appointments and dispatch colleagues for in-home services and on outings.

Available 24/7

We are available 24/7 and our systems alert us when there is a fall or when security is breached. Family members have access to our Smart Hub from their mobile phones and can be fully engaged throughout the time their loved ones are under our care.     

Our Digital Kit

Vivabrook has further adopted proven, value-added technology which significantly increases the operational efficacy of our services. Our Digital Kit, a proprietary assembly of minimally invasive remote monitoring devices specific to the needs of seniors is an integral component of our care programs. Vivabrook also deploys a cloud based Electronic Health Record (EHR) platform, caregiver onsite apps and digital wound care monitoring, all of which operate seamlessly and in full compliance with local regulatory requirements.

Activities of Daily Living (ADL)

We help with ADLs with the bulk of our services centred on the following :

  • Provision of 3 meals a day
  • Medication Monitoring
  • Personal care, including bathing, dressing and grooming
  • Incontinence
  • Mobility including transferring and toileting
  • Housekeeping and laundry
  • 24-hour emergency care
  • Skilled nursing services and selected medical services
  • Social and recreational activities along with companionship
  • Light exercise programs

We regularly monitor the ADL requirements of seniors up to the end-of-life. Our services accommodate unscheduled events such sudden illness or the onset of chronic conditions. In addition, we invite family members to have regular meetings to share how their loved ones are doing and what more can be done. Family members can also initiate meetings whenever they see a need.

Parkinson’s and Dementia

Parkinson’s and Dementia are neurocognitive disorders which progressively require more assistance with ADLs which we anticipate and provide. In addition, seniors are encouraged to participate in specially designed exercise programs and physical therapy as well as puzzles and games which are known to promote cognitive stimulation.

The Movement Disorders Clinic at UBC Hospital and UBC’s Pacific Parkinson’s Research Centre are dedicated to the diagnosis, management and research of Parkinson’s. In the event there is interest in participating in their programs and clinical trials, or those of other similar centres, our Concierge Care Team will facilitate this process. 

Lifestream™ Retrofit

Most home environments are not age-friendly and adaptation to meet the evolving needs of seniors is necessary. Accessibility and safety when navigating walkways, and the use of bathrooms and toilets are key concerns.

Also included would be the installation of walk-in showers, shower seats, grab bars, and anti-slip flooring including the widening of doorways.  Modification to lighting, kitchen and laundry appliances as well as installation of stairlifts, and transfer equipment may also be needed.  Costs can escalate significantly. Nevertheless, Government grants and loans are available for qualified retrofits if required.

An experienced member of our Concierge Care Team can steward this process and consult with loved ones. 

Companionship

Loneliness often afflicts seniors as loved ones frequently need to cope with the exigencies of life and work.  However, the emotional toll on seniors can be alleviated by an attentive and listening ear; occasional trips down memory lane by visiting a favorite park, café or mall and meeting old friends in the community.  The Concierge Care member assigned to the senior, would be a familiar face with sufficient rapport and trust to assist in selecting actionable items from a bucket list.

Respite Care

Vivabrook Health’s respite services offer short term, person centred support for seniors so that family and informal caregivers can take a needed break from the emotional and physical demands of caregiving while knowing their loved ones are safe and well cared for. Stays are time limited and pre planned,and may also be used during periods of increased stress, transition, or caregiver absence, with the goal of helping the individual maintain function and return home or to their usual living environment.
During a respite stay, seniors receive assistance with personal care, medications, daily activities, and social engagement in a comfortable setting that emphasizes dignity, safety, and continuity of routines, while caregivers gain temporary relief so they can rest, attend to personal matters, or prevent burnout.

Wound Care Monitoring

Our VivaCare™ Concierge Team Members obtain 3D high resolution wound images capturing size and depth using their smartphones to be uploaded and processed on a proprietary Digital Wound Management Platform used by Vivabrook under license. Following processing, AI enabled analytics will track and report to wound physicians  on the progress of wound healing for further treatment options. This method significantly reduces errors in manual measurement as well as exposure to malodors.

This  innovation in wound diagnostics and targeted treatment closes the loop on wound management in tandem with the tracking of wound healing using measurements from 3D digital imaging.

Equally important is that the tracking and treatment of the wounds are administered by Vivabrook’s Concierge Care Team. It is Vivabrook’s mission to add value to its services by investing in leading edge innovation to discover signature treatments for the relief and elimination of chronic diseases which can be administered by members of its care team. 

Chronix™ Wound Innovation

Accelerated Chronic Wound Healing

Vivabrook is developing an innovative handheld device powered by an optical sensing technique for use by our Concierge Care Team members with experience in wound care to identify pathogenic  bacteria in chronic wounds before eliminating them with lethal specificity.

As a first step, a Care Team member will apply a dressing on a chronic wound with a specially configured intermediate layer on which emissions from the wound bacteria will be adsorbed. Next, the intermediate layer will be removed for processing by the handheld device resulting in the harvesting of unique spectral fingerprints of the pathogens for transmission to a spectral library to be matched and identified within minutes along with treatment options. These pathogens can then be eliminated under the oversight of wound physicians and nurses. This occurs within hours after biofilm debridement and before regrowth with the rise of more virulent mutations of bacteria.
